There is a particular kind of dental appointment that changes a person’s relationship with arithmetic.
It is the one where the dentist leans back, sets down the X-rays, and explains that the situation has
progressed past the point where saving individual teeth makes financial sense. What is needed, he
says, is an All-on-4. A full arch replacement. Fixed teeth on four implants, same day, permanent.
Then he hands over the treatment plan.

What All-on-4 Actually Is
The name is the procedure. Four titanium implants are placed at precise angles in the jawbone,
providing enough structural support for a full fixed bridge of teeth. Unlike traditional implants
which require one implant per tooth, All-on-4 uses four strategically positioned posts to support an
entire arch. Unlike dentures, the result does not move, does not require adhesive, and does not come
out at night.
The patient arrives with failing teeth, broken teeth, or a removable denture that has been making
mealtimes an exercise in careful management for longer than they would like to admit. They leave
with a fixed temporary arch in place, same day. Three to six months later, once the implants have
integrated with the bone, the permanent bridge goes in.
It is, for patients who qualify, the most complete solution available. It is also, in Western European
cities, the most expensive.

The Arithmetic of Going Abroad
Albania, specifically Tirana, has spent the last decade building a dental sector aimed squarely at
Western European patients who have done the arithmetic and found it wanting at home.
The flight from London takes two hours and forty-five minutes. From Munich, one hour thirty.
From Milan, one hour forty. Direct routes operate from most major European airports. Return flights
run from GBP 80 in low season.

have looked at the saving — GBP 8,000 to GBP 11,000 on a single arch, significantly more on both– and made a rational decision that EUR 3,990 in Albania and EUR 3,990 in London are not the same number even when the implants are identical.
The typical patient is between 50 and 70, initially skeptical, referred by someone who has already
been, and convinced once they verify the implant brand and read the warranty terms. The skepticism
about Albania as a destination tends to resolve on landing. The airport is modern. The hotel is four
stars. The clinic is spotless. The food is better than expected.

The Honest Accounting
All-on-4 is not for everyone. For patients who need one or two individual implants the calculation is
different. For patients who need a full arch or both arches replaced, the saving available in Tirana
compared to any Western European city is large enough that the more interesting question is why
more people are not already on the plane.
